The new Ontario Personal Service Setting Regulation (Reg. 136) introduced on July 1, 2018 by the Ontario Ministry of Health and Long-term Care, means changes for all Personal Service Setting (PSS) Owners/Operators.

If you are, or you think you are providing a hands-on personal service to the public, help us confirm that we have your information and that it is up-to-date by registering with Lambton Public Health.

These changes will help ensure minimal public risk of contracting blood-borne and other types of infections.

Complete the Owner/Operator Reporting Form:

  • Website Form - direct update to Lambton Public Health
  • PDF Form - complete the form, email or print and provide to Lambton Public Health
