According to the Ontario Immunization of Schools Pupils Act, RSO 1990, the County of Lambton Lambton Public Health must keep the vaccination record of every child attending school in Lambton County. The vaccine record is required for school attendance.
When your child receives a vaccination at a doctor's office or a hospital, it is the responsibility of the parent/guardian, NOT the doctor's office or school, to provide Lambton Public Health with your child's vaccination information. To help you provide this information, several options are available to report the information:

To complete the online immunization reporting form, scroll down the page.
Fill in the boxes beside the vaccine name and the date the vaccine was given. When you are finished, click "submit."
