Options to report immunizations:

According to the Ontario Immunization of Schools Pupils Act, RSO 1990, the County of Lambton Lambton Public Health must keep the vaccination record of every child attending school in Lambton County. The vaccine record is required for school attendance.

When your child receives a vaccination at a doctor's office or a hospital, it is the responsibility of the parent/guardian, NOT the doctor's office or school, to provide Lambton Public Health with your child's vaccination information. To help you provide this information, several options are available to report the information:

Completed forms can be:

  • Faxed to 519 383-7092
  • Mailed or dropped off to Lambton Public Health (160 Exmouth St., Point Edward ON, N7T 7Z6)

Information required:

  • Child’s name
  • Gender
  • Date of birth
  • Ontario Health Card number
  • Doctor or Health Care Provider
  • Vaccine(s) received and date(s)
  • School (if applicable)
  • Day Care Facility (if applicable)
  • Parent or guardian's information (First and last name, mailing address
  • phone number)

* All pupils entering nursery, elementary and secondary schools must be immunized against DIPHTHERIA, TETANUS, POLIO, MEASLES, MUMPS AND RUBELLA ACCORDING TO THE DAY NURSERIES ACT, 1984, AND THE IMMUNIZATION OF SCHOOL PUPILS ACT 1990. Exceptions to this requirement are for conscience or religious beliefs (must have a signed affidavit) or medical reasons certified by a physician.

The personal information on this form is collected under the authority of the Health Protection and Promotion Act, R.S.O. 1990, c. H.7. This personal information may be shared with the Ontario Ministry of Health, Ontario Ministry of Community & Social Services, your child's school and/or your local Health Department should you move out of Lambton County.
