Reporting Immunizations

According to the Ontario Immunization of Schools Pupils Act, RSO 1990, Lambton Public Health must keep vaccination records for every child attending school in Lambton County. The vaccine record is required for school attendance.

When your child receives a vaccination at a doctor's office or a hospital, it is the responsibility of the parent/guardian, NOT the doctor's office or school, to provide Lambton Public Health with your child's vaccination information.

Lambton Public Health staff will review all reported immunizations submitted. If there is a discrepancy or additional proof of immunization is required the parent/guardian will be contacted.

Options to report immunizations:

Reporting information required:

  • Copy of child's immunization record
  • Child’s name
  • Gender
  • Date of birth
  • Ontario Health Card number
  • Doctor or Health Care Provider
  • Name of School or Day Care
  • Parent or guardian's information (first and last name, mailing address, phone number)
