If you suspect or have confirmation of a reportable disease, report it immediately to the local Medical Officer of Health. Timely reporting of communicable diseases is essential to monitor the health of the community and to provide the basis for preventive action.

Reportable Diseases

The following diseases listed at this link are specified as reportable diseases under the authority of the Health Protection and Promotion Act, Ontario Regulation 559/91. A PDF resource is also provided under Downloads, for viewing and printing with contact information for Lambton Public Health.

Required Reporting

The following individuals are required to report to Lambton Public Health:

  • All Physicians 
  • Members of the College of Chiropractors of Ontario; 
  • Members of the Royal College of Dental Surgeons of Ontario; 
  • Members of the College of Nurses of Ontario; 
  • Members of the Ontario College of Pharmacists; 
  • Members of the College of Optometrists of Ontario; 
  • Persons registered as a drugless practitioner under the Drugless Practitioners Act; 
  • Hospital Administrators; 
  • School Principals; 
  • Laboratory Operators; and 
  • Administrators of institutions including, but not limited to: 
    • Day Nurseries; 
    • Homes for Special Care; 
    • Long-Term Care Homes; 
    • Psychiatric Facilities; 
    • Correctional Institutions; 
    • Detention Facilities; and 
    • Private Hospitals
